Shale vs. Gravel as base?

Yes, shale binds better, but by selecting course, bank run gravel over rounder runner crush gravel you can find big, bony stones that will lock. Another opinion was that more important than drainage of the base, was the ability of shale to hold the crown of the road, so that water won't gather.

Gravel Driveway Repair

Yes, fill in the pot holes first either by grading the entire width of the drive or just fill the holes with well graded crushed stone from 3/4" to 1-1/2" in size. Lay the geotextile fabric and place 6"-8" of crushed stone base course on top of it.

Shale as driveway base

The escavator proposes putting down Mirify on top of the initial base, then another 6-12" base of only shale (which I have on my property for free) on top of mirify, then 1-2" of runner crush or millings in the spring. So really, the final driveway won't be …

Using recycled concrete and asphalt

They say their crusher can crush the pieces into 6 inch pieces. My thoughts are that this would be fine provided there were enough fines in the material and treat it like a shot rock fill. Density of 3" crushed rock

Bear in mind coarser soils have a lower void ratio than finer soils. So, just because you can see the voids of a large aggregate material, does not mean the void ratio is greater than a fine grained soil. Given a void ratio of 0.3 and a specific gravity of 2.7, the dry density would be: GammaD=0.7*62.4*2.7=118 pcf
